Day-Training (DT)

This service supports the SCL recipient to participate in daily meaningful routines in the community, and it stresses training in the activities of daily living, self-advocacy, adaptive and social skills, and vocational skills. On-site services should lead to the acquisition of skills and abilities for work and/or community participation. Off-site services are provided in a variety of community settings, promoting inclusion and natural supports.

Assessment Services

Comprehensive assessment service which shall identify the needs and services that the SCL recipient, or his/her family, cannot manage/arrange on his/her behalf.  It will also be used to evaluate an SCL recipient’s physical health, mental health, social supports, and environment.  This assessment will be conducted within 7 calendar days of receipt of a request for assessment by an individual requesting SCL services or a family member/legal representative of the individual.  The assessment will include at least one face-to-face contact with the SCL recipient and, if appropriate, his/her family by the assessor in the SCL recipient’s home.

Reassessment Services

Reassessment services will be performed at least annually to determine the continuing need for SCL waiver services using the same procedures as assessment services.  Reassessment will be conducted by a Case Manager or support broker and submitted to the department no more than 3 weeks prior to the expiration of the current level of care certification to ensure that certification is consecutive.  Reassessment is not retroactive.

Behavioral Support

Because some individuals act differently, they may not always be accepted by others. Behavioral Supports are ways to enable individuals to make changes in their lives or environment so others will accept them.  These supports are provided to assist the SCL recipient to learn new behaviors that are directly related to existing challenging behaviors.  It is the use of a planned systematic application of techniques and methods to influence or change a behavior in a desired way and is based on the belief that a behavior is learned and maintained because of consequence.  Also, it is a technique used to increase positive behavior and decrease undesirable behavior.  All behavior support plans will be developed by a behavioral specialist.  The behavior support plan will be designed to equip the recipient to communicate needs and participate in age appropriate activities; prohibit the use of Prone and Supine restraints; and provide for the design, implementation, and evaluation of environmental modifications.

Case Management

The person who helps the individual and/or his/her family to develop a plan to meet the individual's needs is called the case manager. The case manager also works closely with the individual to make sure he/she is pleased with the services he/she receives.  This includes: working with an individual and others identified by the individual, such as family to develop Plan of Care and Services, utilizing person-centered processes,  including planning, to identify and implement support strategies, and working closely with an individual to assure his/her ongoing satisfaction with the process and outcomes of the supports services and available resources.  A Case Manager also deals with coordination, planning, and monitoring progress; however is not a provider of direct services such as transportation to the doctor.

Children’s Day Habilitation

This service provides support, training, and intervention in the areas of self-care, sensory/motor development, daily living skills, communication, and adaptive social skills.  This service promotes inclusion and assists the individual in accessing community resources.   

Community Living Supports

Training or assistance to an individual who does not live in one of the residential supports mentioned earlier. This can include a variety of activities such as shopping, household care, and taking part in community activities, as long as the individual is being taught skills during the activities.  These supports will be provided to individuals in their own homes and will facilitate independence and promote integration into the community for the individuals.  The supports include:

  • Activity training
  • Laundry
  • Routine household care
  • Activities for daily living
  • Personal hygiene
  • Meal planning
  • Shopping
  • Budgeting
  • Medication management
  • Socialization and relationship building
  • Leisure choices
  • Participation in community activities
  • Therapeutic goals

Staff will also be responsible for providing non-medical care to individuals not requiring nurse or physician intervention.  Community Living Supports shall not replace other work or day activities and be limited to sixteen (16) hours per day alone or in combination with adult day training, supported employment, and children’s day habilitation.

Occupational Therapy

The therapeutic use of self-care, work, and play activities to enhance development and achieve as high a level of independence and quality of life as possible. This may include adaptation of tasks or environment to achieve maximum independence and to enhance quality of life. A licensed Occupational Therapist, or an Occupational Therapy Assistant supervised by a licensed Occupational Therapist may provide this support.

Physical Therapy

The use of therapeutic exercise, cardiovascular endurance training, and training in self-care to alleviate pain and improve functional ability. A licensed Physical Therapist, or a certified physical therapist assistant supervised by a licensed Physical Therapist, may evaluate and treat people with health problems resulting from injury or disease.

Psychological Services

The administration of psychological tests for diagnosis, evaluation, and treatment, as indicated by the individual. Dually diagnosed individuals may need this service to coordinate treatment for mental illness and psychological conditions such as emotional instability.

Residential Supports

Residential support options include a family home, adult foster care, staffed residence (a small individualized home), or a group home. Residential Supports provide 24-hour supervision, and training in activities such as laundry, routine household care, self-care, shopping, money management, socialization, and leisure activities. No more than three individuals receiving waiver services can live in any residential setting.


A time of rest or relief for those persons normally caring for an individual. This service is only available to an individual living in a family home residence, and is not for those living in a group home or staffed residence.  The SCL recipient must be unable to independently administer self-care.  Respite services are furnished on short-term basis because of the absence or need for relief of those persons normally providing the care.  Services will be provided in a variety of settings including, but not limited to an individual’s home or place of residence, certified family homes, or the home of an approved respite provider. 

Respite hours will be limited to one thousand four hundred and forty (1440) hours per calendar year.  The hours will be provided to only an individual living in a family home or their own residence.

Speech Therapy

Speech therapy assists an individual in improving communication and is provided by a licensed professional.  The service includes an evaluation of an individual with speech or language deficits in an effort to determine and recommend appropriate speech services.  Therapy also includes a provision of necessary habilitation services for an individual with speech and language disabilities.

Supportive Employment

Intensive, ongoing support for an SCL recipient to obtain and/or maintain paid employment in an environment in which an individual without a disability is employed.  These services include paid employment for persons where competitive employment at or above minimum wage is unlikely and who need intense ongoing support to perform in a work setting because of a disability.  Supported Employment services must be provided by a certified job coach.  To become a certified job coach, staff will complete training by OVR, HDI, DMR, or training conducted by a trainer who has successfully completed DMR training of trainer for supported employment.

Supportive Employment is conducted in a variety of settings, particularly in work sites in which persons without disabilities are employed.  Activities needed to sustain paid work by an individual receiving waiver services, including supervision and training; and paid when not available under a program funded by either the Rehabilitation Act of 1973 or P.L. 99-457.
